To compare the performance of the short (GDS-S) and long (GDS-L) forms of the 20-item Geriatric Depression Scale (GDS) in a community sample of patients with dementia, and to examine the utility of the GDS as a measure of depression in this population. Participants (N = 556; age range: 50-92 years; mean age = 69.4 years; 70.7% females) were drawn from a community-based study of risk factors for dementia. A cross-sectional evaluation of the performance of the GDS in patients with dementia and a detailed clinical characterization of patients with depression. A cut-off score of 5 on the GDS was highly sensitive (94.4%) and specific (89.6%) for the diagnosis of depression in patients with dementia. Patients with dementia with major depression, as defined by the DSM-III-R criteria, had lower scores on the GDS (mean score = 9.0) compared with patients with other types of depression (mean score = 9.8) and patients with no depression (mean score = 9.7). Performance on the GDS was associated with scores on the cognitive screening tests, but not with sex, age, education, diagnosis of mixed dementia, or presence of a caregiver.
